If you have any questions regarded your co-payment, deductible or other responsibility with using health insurance for services such as Chiropractic. Simply give us a call and we would be more than happy to look this up for you!
AUTO INJURY
We gladly accept all open auto accident claims at no cost to you. Simply have your claim number, adjuster’s name / phone number & date of accident handy for us to look up benefits. If you have any prior studies (e.g. MRI, X-Ray, Medications) performed as a result of the injury, please bring them with you on your first visit.
WORK INJURY
Injured at work? We have you covered! Simply give us a call with your claim number, adjuster’s name / phone number & date of accident so we can verify that our office is on your work’s panel. If the injury is longer than 90 days, a panel check will not be needed as you can be seen by any physician of your choosing.

Have a HSA or FSA? Know the difference!? Feel free to use this for everything from co-payments to our cash rates listed below. If you have a FSA that expires each year, feel free to use it towards pre-paid visits so you don’t lose it!
